Caregivers Alberta is part of a new approach to healthcare called Social Prescribing. Social Prescribing focuses on how the social factors of health can impact patient well-being, and the treatments prescribed aren’t always medications. Partnering with Healthy Aging Alberta and other community agencies, healthcare professionals refer patients – and their caregivers – to community-based programs to improve their health and enhance their quality of life.
Social prescribing promotes health equity by addressing social needs that affect health and well-being – like access to adequate housing, food, transportation, income and social support.
Our Navigator works directly with caregivers to assess what resources they may need and then locates and connects the caregiver to those services. If you are a caregiver struggling to find community resources, we can help.
