Urinary Incontinence Information Survey

Urinary incontinence (UI) can affect many aspects of daily life, including physical comfort, emotional well-being, social activities, confidence, and quality of life. Through this confidential survey, Dr. Cheryl Okoli is gathering information to better understand how individuals, caregivers, and health care workers address urinary incontinence from a holistic perspective.

Your experiences and insights can help support future education, awareness, and practical solutions that improve the lives of seniors living with UI.

Participation is voluntary, and all responses will remain strictly confidential.

Thank you for taking the time to contribute to this important initiative.

For any questions on this survey, please reach out to Dr. Cheryl Okoli
